Skills for Health Core Skills Training Framework (UKCSTF).

Question: What is the Skills for Health's Core Skills Training Framework (CSTF) and should it be adopted by all healthcare and social care organisations in the UK?

Answer: The Skills for Health's UK Core Skills Training Framework (CSTF) was developed to provide a national standard for the education and training of healthcare professionals across the UK. The CSTF is now widely recognised across the healthcare sector by NHS England and NHS Employers, Health Education England (HEE) and professional bodies such as the British Medical Association (BMA) and the Nursing and Midwifery Council (NMC), the Health and Care Professions Council (HCPC) and by health and social care regulators, such as the Care Quality Commission (CQC) in England.

Skills for Health's UK Core Skills Training Framework (UKCSTF)

The Core Skills Training Framework was originally designed to cover the statutory and mandatory subjects that are essential for safety and compliance with the law, the SfH framework has been extended to include clinical and care subjects. The framework can be used by any healthcare employer in the UK, including primary care. It can provide the basis for an organisations’ training policy, especially their policy relating to statutory and mandatory training, such as manual handling, infection control, fire safety training and resuscitation.

The framework clarifies what all providers need to be delivering.A suite of eLearning courses is available to enable staff to complete the necessary training to comply with the framework, with specific primary care versions for general practice staff.

Details of the training available can be found in the SfH publication, Elearning — Cost-effective, Quality-assured Courses for Essential Training.The web-based resources can be launched through the National Skills Academy for Health who host the courses.

Progress and test results can be tracked by managers for internal development purposes through the Workforce Information Reporting Engine Database (WIRED) self-service reporting tool. These training records can be used during regulatory inspections to provide assurance to inspectors that any required standards of training are being met. For example, in England this includes training required under the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 regulated by the CQC.

Many primary care employers find the framework especially useful to ensure compliance with statutory and mandatory training. This is training that is required by law or to comply with mandatory requirements in organisations or sectors, such as the NHS. Adopting the framework also makes statutory and mandatory training easily transferable. It allows staff to move from organisation to organisation with their training record intact and an assurance that common standards are being used. This saves staff and organisations from wasting time and money in repeating training unnecessarily.

Healthcare providers who have mapped their in-house training to the CSTF are eligible to submit a Declaration of Alignment, confirming which of their training programmes are aligned to the framework subjects.

For each subject, detailed learning outcomes, standards and guidance are available in a number of framework documents (eg Subject Guides and Mapping Tools). The framework is fully mapped to the NHS Agenda for Change Knowledge and Skills Framework (KSF).

 

A suite of National Competencies based upon the CSTF has also been added to the NHS Electronic Staff Record (ESR).Full information about the framework is available from the Skills for Health website.

Statutory/Mandatory Core Skills Training Framework (CSTF)

With a highly mobile workforce, employers often need to identify recruits’ previous training. Where information is lacking, people can repeat training unnecessarily, wasting time and money. The Statutory/Mandatory CSTF addresses this issue. It allows employers to recognise statutory/mandatory training to a standard, trust that information and reduce duplication of training. Launched in 2013, the Statutory/Mandatory CSTF is maintained and updated by Skills for Health.

Alignment to the Statutory/Mandatory CSTF

The Care Certificate

The Care Certificate is an identified set of standards that health and social care workers adhere to in their daily working life. Designed with the non-regulated workforce in mind, the Care Certificate gives everyone the confidence that workers have the same introductory skills, knowledge and behaviours to provide compassionate, safe and high-quality care and support.

Other Core Skills Frameworks

Other core skills frameworks are offered as guidance and good practice for the development of the current and future workforce. Unlike the Statutory/Mandatory CSTF, there is no formal process for declaring alignment to these other frameworks.

Each framework sets out the core capabilities – the knowledge, skills and behaviours needed – for safe and effective practice. T

Dementia (2015, updated 2018)

The original Dementia Core Skills Education and Training framework was launched in October 2015. In June 2018 a revised and updated version was released, entitled the Dementia Training Standards Framework.

The framework was commissioned and funded by the Department of Health and developed in collaboration by Skills for Health and Health Education England (HEE) in partnership with Skills for Care. Development of the framework was guided by an Expert Group including a wide range of health and social care organisations, relevant Royal Colleges and education providers.

The framework sets out the core skills and knowledge which would be transferable and applicable across different types of service provision. It includes the expected learning outcomes for training delivery, key policy and legal references and is aligned with related national occupational standards.

The aim is to help ensure the quality and consistency of dementia training and to help prevent unnecessary duplication of training. The framework comprises the following subjects:

  2. Dementia identification, assessment and diagnosis
  3. Dementia risk reduction and prevention
  4. Person-centred dementia care
  5. Communication, interaction and behaviour in dementia care
  6. Health and well-being in dementia care

Person-Centred Approaches (2017)

Health Education England commissioned Skills for Health in collaboration with Skills for Care to develop this core skills education and training framework for person-centred approaches.

The value of a person-centred approach in health and care is increasingly recognised - much has been written about what it is and why it is important. However, it is less clear how to develop the workforce to put this into practice – which is the purpose of this framework.

The framework launched in July 2017 and comprises the following subject areas:

  • Values
  • Core communication and relationship building skills
  • Step 1. Conversations to engage with people
  • Step 2. Conversations to enable and support people
  • Step 3. Conversations with people to collaboratively manage the highest complexity and significant risk
  • Delivery of training, education and learning opportunities
  • ‘Enablers’ for embedding a person-centred approach.

End of Life Care (2017)

Health Education England commissioned Skills for Health in collaboration with Skills for Care to develop this Core Skills Education and Training Framework for End of Life Care.

Launched in March 2017, the framework expands on the Common Core Principles and Competencies for Social Care and Health Workers Working with Adults at the End of Life and aligns with the current Dementia, Mental health and Learning Disabilities frameworks all of which will sit under an overarching framework for Person-Centred Care.

In particular, it aims to promote community development and asset-based approaches to education and training, considering the strengths, abilities and potential of communities in supporting the end of life care. The framework comprises the following subjects:

Mental Health (2016)

The Mental Health Core Skills Education and Training Framework was commissioned and funded by the Department of Health and developed in collaboration by Skills for Health, Health Education England and Skills for Care.

The framework supports workforce development specific to the implementation of the national mental health strategy, No Health Without Mental Health. The aim of the framework is to support the development and delivery of appropriate and consistent cross-sector mental health education and training.

The framework describes core skills and knowledge i.e. that which is common and transferable across different types of service provision. The framework covers the following subjects:

Learning Disabilities (2016)

The Learning Disabilities Core Skills Education and Training Framework was launched in July 2016. The framework was launched by Skills for Health in partnership with Health Education England and Skills for Care.

The framework sets out the essential skills and knowledge necessary for all staff involved in learning disability care.  The framework covers the following subjects:

WHICH TYPE OF MANDATORY TRAINING DO I NEED?

Most agencies and employers now insist that healthcare professionals complete Mandatory and Statutory Training aligned to the Skills for Health UK Core Skills Training Framework. The content of this training course has been verified by Skills for Health as aligning to the UK Core Skills Training Framework.

You can still do the ‘All in One Day’ Mandatory Training, however, it is not aligned to the Skills for Health UK Core Skills Training Framework. We recommend that you now choose the 'Skills for Health Aligned CSTF' Mandatory + Statutory Training but please talk to your employer about your options first.

WHO IS MANDATORY TRAINING FOR?

Care home statutory and mandatory courses

Care homes provide treatment to vulnerable individuals and the quality of care provided is largely determined by the staff within the service – many of whom are care assistants. The Care Standards Act 2000 (standard 28) requires three days of annual training for care home staff.

Mandatory Training For Care Homes – the requirements

Annual mandatory training courses must include fire safety, moving and handling, and health and safety. There is also an expectation that every shift is covered by at least one person who has been trained in first aid. This can be a huge but necessary expectation of both the staff and the home.

Which Training Courses are Mandatory for Care Homes?

So, which training courses are mandatory for care homes and care home staff?  – There is no straightforward answer ton this question. Ultimately the requirements will vary depending on the size of the care home, its services and outcome from its own risk assessment on site.

Regulation 23 of the Health and Social Care Act 2008 states:

The registered person must have suitable arrangements in place in order to ensure that persons employed for the purposes of carrying on the regulated activity are appropriately supported in relation to their responsibilities, to enable them to deliver care and treatment to service users safely and to an appropriate standard by:

  1. Receiving appropriate training, professional development, supervision and appraisal; and
  2. Being enabled, from time to time, to obtain further qualifications appropriate to the work they perform.

Outcome 14 is the outcome that CQC uses to ensure service meets the requirements stipulated in regulation 23 and this states:

People who use services:

  • Are safe and their health and welfare needs are met by competent staff

This is because providers who comply with the regulations will:

  • Ensure that staff are properly supported to provide care and treatment to people who use services.
  • Ensure that staff are properly trained, supervised and appraised.
  • Enable staff to acquire further skills and qualifications that are relevant to the work they undertake.

The common mandatory courses for a care home would be:

This list could also include the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS) training. Fire safety training is not mentioned within this list as this would result, where required, from the organisation's statutory Fire Risk Assessment (FRA).

These courses also reflect and address many of the Common Induction Framework objectives.

Professional registration and development requirements relate to Medicines Administration for Qualified Nurses and Supervision, care planning and record keeping training.

The key is to enable staff to take part in learning and development that is relevant and appropriate so that they can carry out their role effectively.

The Care Quality Commission

The role of the Care Quality Commission (CQC) is to check whether hospitals, care homes, GPs, dentists and services registered with them are meeting national standards.

Their website also offers guidance on the requirements and you should refer to these to ensure you are monitoring your compliance to the required standards.

Good standards of health and safety in the workplace do not happen of their own accord. Safe systems of work have to be devised and implemented, staff have to be trained and equipment needs to be purchased and maintained.
